Recreation and Community Services Unclaimed Credit Form

Pursuant to Section 50052 of the Government Code of the State of California, I (we) would like to file a claim for a previously unclaimed credits that was published in the Milpitas Post on April 18, 2025.

Per CA Government Code Section 50052, the monies reported in the Milpitas Post will become the property of the City of Milpitas beginning June 18, 2025, unless a claim is perfected.

Please note: All information must be filled out completely and proof of residency must accompany all requests. If any information is not included your request will be denied and you will have to re-file another claim.

The grounds on which this claim is founded are as follows:
Upload proof of residency. Acceptable proofs include:
1) A current CA Drivers license or CA issued ID; OR
2) Utility Bill, Financial Statement (i.e. PG&E, AT&T, Bank, etc.)
